Admission requirement for the program is as follows:

• 24 years of age or older or 18-23 and eligible for the Chronic Illness Initiative
• High school graduate
• English language proficiency

Successful completion of Learning Assessment Seminar (see below)

In order to apply, you must follow these steps.

You must first fll out an application form. The application form can be downloaded from http://www.snlonline.depaul.edu/pdfs/Application.pdf and can be faxed to 312-362-8809.

You will be fully admitted into the program once you complete the Learning Assessment Seminar requirements (see below).

All prior college transcripts should be sent directly to DePaul in a sealed, official envelope. It is important that you also request a copy for your records as well, you will need this during the Learning Assessment Seminar. If you have not attended college in the past, please request that a copy of your high school transcripts be sent to DePaul.

Learning Assessment Course

You will be fully admitted into the program if you successfully complete the Learning Assessment Seminar and submit all the required paperwork, including transcripts from prior institutions. LAS is a four-week introductory course for which you will receive two credits and one competence. Registration for this seminar is open now. Follow this link to register for the LAS Seminar.
